Institute of Ophthalmology could be UCL's, but I have not been able tro confirm that.
To find Duke-Elder's academic parents, I searched University of St Andrews's library catalog for Duke-Elder's various theses, hoping to see their Acknowledgements sections. The only thesis I could find was his 1927 DSc; it is not available on-line.
I also read two of Duke-Elder's publications prior to 1927. The contained no Acknowledgements sections.

To find Duke-Elder's academic children, I searched UCL's library for theses Duke-Elder might have supervised, but there seems to be no way to identify supervisors from UCL's library's search tools. More than 200 theses, from ProQuest, contain citations of Duke-Elder's works, but this is not particularly helpful.
