Consortium Therapeutic Studies of Cns Malignancies @ University of California Los Angeles DESCRIPTION (provided by applicant): UCLA Neuro-Oncology Program will participate in the clinical trials of the North American Brain Tumor Consortium (NABTC) as a "Member Institution". Member Institutions of the NABTC will also include, the University of Wisconsin, the University of Pittsburgh, Dana Farber Cancer Center, M D Anderson Cancer Center, the Neuro-Oncology Branch at the National Institutes of Health, the University of California, San Francisco, Memorial Sloan Kettering Cancer Center and the Pharmacokinetics Center at the University of Texas San Antonio. UCSF will be the Lead Institution and Dr. Prados the Group Leader for the NABTC. The focus of this grant will be to perform phase I and II clinical evaluations of promising new therapeutic agents or approaches for the treatment of primary CNS malignancies in adult patients, especially glioblastoma multiforme and other high grade gliomas. Additionally, we will perform ancillary laboratory studies of aspects of CNS tumor biology with potential clinical implications. UCLA will continue to extend it ongoing work with NABTC by interacting with other members of the consortium and with NCI in a concerted way to conceive, create and evaluate new approaches to the therapy of CNS tumors. This will be largely achieved through the existing environment and expertise that exist at UCLA including physician investigators, multidisciplinary approach, data management resources, patient population, expertise and advanced instrumentation for neuroimaging, surgery and radiation, and drug control procedures. UCLA will also provide an effective environment and expertise for multi-institutional protocol concept development and protocol conduct. To characterize the safety profile of the doublet combinations of R115777-BAY 43-9006, OSI-774-BAY 43-9006 and CCI-779-BAY 43-9006 in patients with recurrent glioblastoma or gliosarcoma. 3. To characterize the pharmacokinetics of these doublet combinations, evaluating single agent pharmacokinetics of each agent then the combination pharmacokinetics to determine drug-drug interactions. Phase II: 1. To determine the efficacy of each of the doublet combinations in patients with recurrent glioblastoma or gliosarcoma as measured by 6-month progression-free survival. 2. To determine the efficacy of each of the doublet combinations in patients with recurrent glioblastoma or gliosarcoma as measured by 12-month survival and objective tumor response. 3. To further characterize the safety profile of each of the doublet combinations. 4. To characterize the pharmacokinetics of each of the doublet combinations in a subset of the phase II patients to further define possible drug-drug interactions. 5. To perform exploratory correlative laboratory studies by examining tissue markers of signal transduction pathways by immunohistochemical analysis using tissue blocks obtained prior to initiation of protocol therapy, either from the time of diagnosis or subsequent tumor resection. 6. For the Molecular Targeted Combinations Correlative (MTC2) Study Initiative: To determine the relationship between tumor and blood biomarkers and clinical outcome of patients treated with the combination of targeted agents. Career Enhancement Program @ University of California Los Angeles CEP: Career Enhancement Program SUMMARY/ABSTRACT A central mission of the UCLA SPORE in Brain Cancer is to stimulate, recruit, develop, and retain new investigators in the field of brain tumor research, with particular focus on translation of scientific discoveries to novel clinical applications. The future of brain cancer research lies in the continued creation and development of translational projects and innovative investigators at major research institutions, as well as an institutional commitment to support outstanding, dedicated scientists in the field. In order for translational scientists and physician-investigators to grow and flourish, funding support and peer mentorship is imperative. The UCLA SPORE in Brain Cancer Career Enhancement Program (CEP) will provide this research support and mentoring resource, allowing junior faculty to transition to independent investigator status with adequate peer-reviewed funding and established investigators to focus or redirect their research to the area of brain cancer. CEP funds will be designated to prepare investigators for careers in translational brain cancer research and enhance their chances of success in the field. Applications will be accepted from MD, PhD, or MD/PhD advanced post- doctoral fellows who are ready to transition to a faculty position within one year, junior faculty, and established investigators to work with a senior mentor, and special efforts will be made to recruit qualified women, individuals from underrepresented racial and ethnic groups, as well as individuals with disabilities. Applications will be reviewed by the CEP Selection Committee, IAB and EAB members, and ad hoc reviewers with particular areas of expertise. Each year, we anticipate supporting 2 to 3 highly innovative candidates with CEP funds. Ultimately, the CEP is designed to increase scientific inquiry and excellence across the continuum of research on the prevention, early detection, diagnosis and precision therapy for brain cancer.
